ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

- Regular and predictable attendance is an essential requirement of this
position.
- As a shift leader, teach, mentor and support Rehab. Tech I staff
understanding, skill and follow-through on resident Care Plans, medication and medical treatment provision and resident support for personal care, supervision and protection.
- Supervise directly or track whereabouts of patients depending on the assessed
level of independence.
- Monitor visitors to facility utilizing individual patient’s Visitor
- Guidelines. Communicate details of visits, both positive and problematic to the
individual’s Social Worker. Forward questions and concerns of members of the individual’s support network to the appropriate team members.
- Report signs and symptoms of health problems by interviewing patients and
taking vital signs. Document utilizing a Health Care Assessment form, Seizure Log or other health record.
- Manage patients’ chronic medical conditions, as prescribed and according to
- training protocols, including Ex. diabetes through application of prescribed
diets, administration of insulin and measurement and response to high and low BSLs. Respond proactively to health crises.
- Escort patients to physician and other community medical and therapy
appointments. Relay pertinent information from the treatment team to the medical
- provider. Communicate and document physician instructions back to the HNNR team
utilizing a Health Care Consultation Form. Submit billing information regarding transportation and supervision time according to corporate guidelines.
- Review and demonstrate knowledge of individual patients’ physical/functional
and cognitive profile, utilizing strength areas to optimize communication, memory and learning. Reference individual Care Plans to match support and intervention techniques to current function.
- Provide therapeutic interventions and treatments as prescribed or according
the objectives established by patient’s therapy team. Utilize adaptive equipment and apply orthotic devices according to physician/therapist instruction.
- Implement patients’ Behavioral Intervention Plans. Document on data
collection tools, including sleep logs and complete behavior scales and other standardized measures, as provided by the individual’s psychologist.
- Escort patients to community leisure activities, Ex. sports activities,
grocery or personal needs shopping, family events. Provide cues and support for optimal independent functioning and psychosocial relationship skills.
- Participate and assist individuals with in-house activities to promote
meaningful use of free time and offer functional growth opportunities. Record activity participation in individual case record Progress Notes.
- Respond/follow up on the concerns/questions of the staff; promote positive
morale, act as moderator in situations of staff dispute or disagreement involving the workplace.
- Provide regular resident advocacy and education. May conduct Resident
Council meetings and complete action related to feedback gleaned.
- Others duties as assigned.
Educational / Talent Requirements:
- Demonstrated ability to communicate in verbal and written formats sufficient
to communicate detailed information, critical incidents, physicians’ instructions, as well as patient/resident participation and response to clinical interventions.
- Six months experience as an HNNR Rehab. Tech I or two years of experience in
a human service agency.
- Demonstrated competency in all HNNR Core Medical Rehab Tech Skills.
- Computer skills including use of Microsoft Office. Ability to learn and use
electronic Medication Records, electronic Medical Records and others as needed.
- Certification or ability to obtain Certification in First Aide/CPR.
- Valid Driver’s license with driving record acceptable according to Hope
Network policy.
- Ability to work independently with flexible hours.
Work Experience Requirements:
- Six months experience as an HNNR Rehab. Tech I or two years of experience in
a human service agency and have completed all HNNR Core and Medical Rehab Tech Skills.
- Work experience in a licensed residential setting preferred.
