We are seeking a community-based Blended Case Manager to join our team in Colmar, PA serving clients in Montgomery County.
The Blended Case Manager is responsible for developing, coordinating, and
- implementing services for adult consumers as identified in the goal-oriented
service plan. This role includes providing case management, service planning, and coordination of services with other agency providers to meet the consumer needs and increase community stability in consumer's life.
The Blended Case Manager will:
- link consumers with a mental health or behavioral health diagnosis to
resources and encourage, teach, and guide them to advocate for themselves. Case Manager will
- complete comprehensive assessments, service plans, and environmental matrixes
to determine level of care and frequency of contact with consumers on their caseload.
- complete other paperwork requirements from OMHSAS, Magellan Behavioral
Health, Montgomery County, and Merakey and will be monitored by Case Manager and completed timely.
- have appointments with consumers in their homes, community, and hospitals and
should be familiar with Montgomery County, including being able to access resources to meet client's needs.
- carry a caseload between 25-30 clients and Case Manager will meet with
clients according to matrix score, either every 14 or 30 days while
- maintaining Merakey's productivity standard.
- attend supervision meeting with Manager monthly and team meetings weekly.
- work typically Monday-Friday 9 am-4:30 pm but shift may vary at times based
on client's needs. This role includes optional on-call.
Earn $22.29 per hour OR $24.29 per hour with the selection of our Enhanced Pay option!

Qualifications:
The ideal candidate will possess the following qualifications:
- Valid driver's license with clean motor vehicle report.
- Basic computer skills and experience with an EMR system preferred.
- Case Management experience with adults and children preferred.
Educational Requirements:
- Bachelor's degree with major course work in sociology, social welfare,
psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or
- Registered nurse; or
- A high school diploma and 12 semester credit hours in sociology, social
welfare, psychology, gerontology, or other social science and two years of
experience in direct contact with mental health consumers; or:
- A high school diploma and five years of mental health direct care experience
in public or private human services with employment as a case management staff person prior to April 1, 1989.
