Requirements:

To be seriously considered for this role, you’ll need to have:

  • EducationMinimum requirement of a Master's Degree in social work from a college/university accredited by the council on Social Work Education. 
  • Experience: Minimum one year experience in an acute care setting; long term and or home health experience preferred
  • Licenses/Certification: LSW or SWC required. Valid Colorado Driver's license within 30 days of hire, current automobile insurance, reliable transportation and current CPR certification

Responsibilities:

We are committed to improving the health of our community by delivering exceptional care and services.  As part of the team in Home Health/Hospice, you will:

  • Evaluate, instruct, observe and treat patients exhibiting significant social and emotional problems affecting their health status.
  • Provide active leadership through the sharing of knowledge and personal maintenance, and enhancement in the area of social services.  Act as consultant to other agency personnel.
  • Monitor pain in patients and follow up appropriately
  • Assist patients and families with long term planning as well as education regarding “5 Wishes” and making end of life decisions.  Involve client as able in decision making and planning of care
  • Prepare clinical and progress notes in accordance with agency policy.     Identify and plan for safety needs of the adult patient and initiates referral(s) as appropriate and necessary
  • Provide information on community support services and contacts appropriate community referrals to arrange for further services as appropriate and as discussed with other healthcare team members.
  • Use policy and procedure manuals, and other reference materials to ensure proper course of action.  Provide accurate and timely information using discretion and protects confidentiality of information.
  • Manage and ensure an organized program for the provision of bereavement services furnished under the supervision of a qualified professional with experience or education in grief or loss counseling
  • Make bereavement services available to the family and other individuals in the bereavement plan of care

Compensation: $28.18 - $34.52/hr, depending on experience.
