Health System Overview:

Gunnison Valley Health is the sole community-owned health system for Gunnison County and surrounding areas. We are not just a hospital, but a fully integrated health system providing services for all stages of life; from a newly renovated Family Birth Center, to a 52,000 square foot, state of the art senior care center and everything in between.  We are a leading health source for our community, offering wellness programs, medical education and illness prevention.  We are looking for a Physical Therapist with proven capability to treat clients for pain relief, and restoration of function to maximum capacity while improving your own professional enrichment and growth by attending continuous education programs.

**Responsibilities: **

Provide direction, supervision and evaluation of PT services to patients in their home in accordance with agency policies & procedures, applicable health care standards and governmental laws and regulations.  Complete initial evaluation visit, regularly re-evaluate the patients PT needs, initiate the plan of care and furnishes services.  Coordinate with physicians of changes in patients condition. 

EducationPhysical Therapist: Diploma BS or greater from an accredited program of Physical Therapy.

Experience: Must have a minimum, one (1) year clinical experience, preferably in home healthcare.

Licenses/Certification:  Physical Therapist: Licensed in the State of Colorado, valid Colorado Drivers license within 30 days of hire, current automobile insurance, reliable transportation and current CPR certification.

Compensation: Physical Therapist: $36.40 - $47.31/hour DOE

Physical Requirements:

Occasionally (1-33%) Reaching, reach across midline, stooping, stairs, lifting (0-20 lbs), carrying (25-50 lbs), pushing/pulling (25 - 50 lbs)

Frequently (34 - 66%) Standing, walking, sitting, change position, handling, pinching, crouching/squatting

Continuously (67 - 100%) Must be able to see with corrective eyewear, must be able to hear clearly with assistance
